January 9, 2003
Medications:
• Trazodone 25 mg
• Celexa 20 mg
• Lorazepam 2 mg
• Depakote 500 mg
• Naproxen as needed
Comments and Complications: Lexapro given up as not suitable, changed back to Celexa and added Depakote twice a day. Naproxen 550 mg being taken pretty much every day, twice a day for body and/or headache pain.

Medications:
• Trazodone 25 mg
• Celexa 30 mg
• Lorazepam 2 mg
• Depakote 500 mg
• Naproxen 550 mg, twice daily
Comments and Complications: Increased Celexa due to continuing depression.

• Trileptal 300 mg
• Naproxen 550 mg, as needed
Comments and Complications: Changed from Depakote to Trileptal in an effort to jump-start weight loss.

• Trazodone 25 mg
• Celexa 20 mg
• Wellbutrin SR 100 mg
• Lorazepam 1 mg
• Trileptal 300 mg
• Naproxen 500 mg, as needed
• Tenoretic 50/25 mg
• Zocor 40 mg
Comments and Complications: Lowered dose of Celexa from 30 to 20 mg and added 100 mg of Wellbutrin SR, hoping this also would help make it easier to lose weight. Also Zocor, taken all along for high cholesterol, doubled when cholesterol hit 235, and Tenoretic added for high blood pressure.

August 1, 2003 (approx.)
Medications:
• Trazodone 25 mg
• Celexa 30 mg
• Wellbutrin SR 100 mg
• Lorazepam 1 mg
• Trileptal 300 mg
• Naproxen 1100 mg
• Tenoretic 100/25 mg
• Zocor 40 mg
• Ambien 5 mg
Comments and Complications: Celexa returned to 30 as tiredness and anxiety over approaching surgery set in. Poor sleep led to adding Ambien; blood pressure medicine increase negated any good effects from these changes. Ambien dropped right away. Surgery August 20th; all sedating medications and Naproxen dropped during hospital stay, resumed upon discharge.

Comments and Complications: Due to depression, Celexa increased to 40, Wellbutrin to 200.

Comments and Complications: Topamax increased to 75, Wellbutrin changed to XL, increased to 300.

November 13, 2003

• Wellbutrin XL 300 mg
• Lorazepam 1 mg
• Trileptal 300 mg
• Topamax 100 mg
• Naproxen 1100 mg
• Tenoretic 100/25 mg
• Zocor 40 mg
Comments and Complications: Topamax increased to 100, Trazodone dropped on trial.
